Health Requirements
in Admission

Students after admission have to submit health documents to the University Healthcare Department which is regulated by the Law of Education as well as the Order of the Minister of Education and Science of the Republic of Kazakhstan of October 31, 2018 No. 600 “On approval of the Model Regulations for admission to studies in educational organization, implementing educational programs of technical and vocational education”.

Documents to be submitted to the University Healthcare Department

I. For local students:

Please prepare the following documents in good quality in PDF format. Please leave original documents at your home in case you need them during the period of study. The original documents are not accepted by the University Healthcare Department.

List of medical documents

Scan version of the original medical certificate form "075-У" (or “086-У”).
Previously, "086/у" was assigned a new number - "075/y" according to the updated order № “ҚР ДСМ-175/2020”. The minimum scope must include the conclusions of the following specialists: otorhinolaryngologist, ophthalmologist, and neuropathologist;

Scan version of the original of a printed chest X-ray (флюорография) or a doctor's report with a seal, valid for one calendar year;

Certificates from
psychoneurological and
narcological organizations
(service is free on www.egov.kz);

Scan version of the original preventive vaccination card form "065-У" or the vaccination passport (preventive vaccination card).
Previously, "063/у" was assigned a new number - "065/y" according to the updated order № “ҚР ДСМ-175/2020”;

COVID-19 vaccination Passport (If any) + booster (if applicable).
If there is no vaccination passport, a scanned version of the medical exemption from vaccination 026-У form (Conclusion of the medical advisory commission) is required;

Scan version of identity document;

For applicants under the age of 18 - list of consent for a medical check-up completed by one of the parents (guardian).
Please fill out the document with blue ink;

For applicants over 18 years of age, the student's list of consent for a medical check-up.
Please fill out the document with blue ink.

II. For international students who do not live in Kazakhstan:

Please prepare the following documents in good quality in PDF format. Please leave original documents at your home in case you need them during the period of study. The original documents are not accepted by the University Healthcare Department.

Health Background Form. Please watch the video explanation on how to fill the Health Background Form;

Scan version of Passport/identity document;

COVID-19 vaccination Passport (If any) + booster (if applicable).

III. For international students who live in Kazakhstan:

Please prepare the following documents in good quality in PDF format. Please leave original documents at your home in case you need them during the period of study. The original documents are not accepted by the University Healthcare Department.

Health Background Form for the foreigners OR scan version of Medical Certificate 075-У
(The minimum scope should include the conclusions of the following specialists: otorhinolaryngologist, ophthalmologist, and neuropathologist.) (with all supplementary documents on vaccination)

scan version of a printed chest X-ray or a doctor's report with a seal, valid for one calendar year, required for those who lives in Kazakhstan;

COVID-19 vaccination Passport (If any) + booster (if applicable).If there is no vaccination passport, a scanned version of the medical exemption from vaccination 026-У (Conclusion of the medical advisory commission) form is required;

Scan version of Passport/identity document.

Note: * (For those international students who live in Kazakhstan):
Please get information about the dispensary observation of the psychoneurological and narcological dispensary through the www.egov.kz e-government website.
After receiving the certificate, contact any medical clinic where a medical check-up is carried out, to issue you a Certificate 075-У, attaching a chest X-ray (The certificate 075-У is on paid basis).
